Read the qualifications, pre-requisites and requirements at http://www.emstar.org/InstructorApp.html and NYS DOH Policies 04-04 and 08-05. All candidates must take the Pre-Screening class.

ENTRANCE REQUIREMENTS*
  • Successful Pre-Screening Exam
  • Completed application (available at http://www.emstar.org/doh2260.pdf)
  • Current NYS EMT certification
  • Written EMS exam within last 3 years, with score of at least 85%
  • Letter from EMS agency captain verifying that you currently provide pre-hospital care, with minimum one year clinical experience within last three years
  • Letter of recommendation from a Course Sponsor
  • If you are in the Pilot Program, contact NYS DOH at 1-800-628-0193 to take a written exam for an instructor's score.
